How to read a bill against the cap or band

In England you should see one band on the treatment plan, not a running total of items. In Wales, Scotland and Northern Ireland you should not be asked for more than the £384 course cap for NHS work in that course. If the number is higher, it is usually private treatment, a new course, or a mix. Ask the practice to split NHS vs private in writing before you sit down. Exemptions still mean £0 when the category applies.

Can a practice charge above the cap?

Not for NHS treatment within the same course. Private treatment is separate and must be agreed in advance.

Is the England Band 3 fee a cap?

It is a fixed course charge of £332.10, not a “up to” cap you might undershoot. You pay that band when the course includes Band 3 work, unless you are exempt.
