England charges a fixed fee per severity band — Band 1 £27.90, Band 2 £76.60, Band 3 £332.10 (2026/27 NHSBSA rates) — one charge per course, the highest band that applies, never one charge per item. Wales replaced its band system on 1 April 2026 with a fixed fee per care package instead (by treatment type, not severity), capped at £384 per course overall. Scotland and Northern Ireland have no fixed fees — you pay 80% of the item cost, capped at £384 per course.
